What is Triptorelin?

Initially patented in 1975 and approved for medical use in 1986, triptorelin is a decapeptide synthetic analogue of gonadotropin-releasing hormone (GnRH) [1].

It is clinically used to treat symptoms of precocious puberty, advanced prostate cancer, breast cancer in premenopausal women, and other conditions off-label [2, 3].

Brand names for this peptide include:

  • Trelstar
  • Trelstar Depot
  • Trelstar LA
  • Trelstar Mixject
  • Triptodur
  • Decapeptyl

As a gonadorelin analog, it works in male subjects by initially triggering a rise (or “flare up”) in follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol, and testosterone. A subsequent sustained decrease in FSH, LH, testosterone, and estrogen occurs two to four weeks from the initiation of long-term triptorelin therapy. In fact, triptorelin can reduce serum testosterone to levels that are typically seen in surgically castrated men [3].

Research has shown that GnRH agonists like triptorelin possess greater affinity for the GnRH receptors than native GnRH, as well as greater resistance to enzymatic breakdown and an extended half-life [4].

Preclinical studies have shown that triptorelin’s effects on spermatogenesis, steroidogenesis, and behavior are fully reversible [5].

Triptorelin Benefits

Triptorelin has exhibited benefits in regards to improving hormonal function and fertility.

Triptorelin can offer a temporary boost in testosterone secretion

Male subjects previously exposed to testosterone or androgenic-anabolic steroid administration will often show decreased levels of endogenous testosterone production after finishing. In this situation, triptorelin may be utilized to reinitiate natural testosterone production.

Due to the fact that triptorelin causes the pituitary gland to secrete follicle-stimulating hormone (FSH) and luteinizing hormone (LH), an increase in testosterone and sperm production can be observed shortly following one-time administration [6, 7].

Triptorelin can stimulate and preserve fertility

Younger subjects who undergo harsh treatments like chemotherapy can face fertility issues. Triptorelin may be administered to protect fertility, as well as reduce the onset of early menopause.

In one study designed to observe the effects of triptorelin administered over the span of 15 years on 36 adolescent females who underwent chemotherapy, 27 patients maintained normal ovarian function [8].

In addition, a study conducted on 150 female subjects suffering from endometriosis and infertility found that triptorelin could assist in achieving pregnancy. Patients were separated into three groups where each was administered a different compound. The results of the study showed that the pregnancy success rate was 53.33% (80/150), with the triptorelin group scoring significantly higher than the other two control groups [9].

Triptorelin can suppress sex hormones

Triptorelin depot is clinically used to treat central precocious puberty (CPP) in children. The peptide’s ability to block pubertal development and slow skeletal maturation allows patients to reach their target height. In addition, a Korean study found that the baseline characteristics of girls treated with a three-month depot were similar to those of the girls treated with a one-month depot [10, 11, 12].

Another study was conducted on 58 French girls and 8 boys treated for CPP with triptorelin, finding that girls with late onset CPP had benefited from GnRH agonist treatment similarly to girls with earlier pubertal onset. This study also showed restoration of genetic target height, as well as a 5- to 10-cm increase in final height in girls treated with triptorelin for CPP [13].

Triptorelin can improve immune system performance

Animal studies have shown that the process of aging is accompanied by a sharp reduction in LHRH-A-binding sites within the thymus gland. As a result, the risk of immune system dysfunction grows with age, thus increasing susceptibility to illness. In one particular study, triptorelin has been shown to reverse age-related decreases in both thymus weight and thymic LHRH-binding sites, suggesting potential therapeutic and preventative applications [14].

Triptorelin Side Effects

Researchers should note that triptorelin may cause a number of adverse effects in test subjects, mainly as a consequence of its shutting down essential hormonal production. However, no long- term adverse events related to reproductive function have been reported [8, 15].

In clinical practice, triptorelin is associated with the following side effects in patients:

  • Painful or difficult urination
  • Chest pain
  • Numbness or weakness
  • Hot flashes
  • Impotence
  • Nausea
  • Vaginal bleeding
  • Cold or flu symptoms

Hot flashes are the most common side effect in men receiving triptorelin for prostate cancer, reported in up to 87% of men.

In a study on Chinese women undergoing fertility treatment with triptorelin, nausea was the most frequently cited adverse event, occurring in 9.2% of patients. Also, 7.5% of patients experienced ovarian hyperstimulation syndrome, which resulted in triptorelin administration being discontinued [4].

Research involving drug-induced liver injury has shown that triptorelin is associated with a low rate of transient serum enzyme elevations during therapy, but has not been linked convincingly to cases of clinically apparent acute liver injury [16].

Triptorelin Dosage Calculator and Guide

In this section, we provide an overview of how researchers may dose and administer triptorelin to test subjects.

There are formally established guidelines for dosing triptorelin in clinical contexts, which are beyond the scope of this article.

Triptorelin is most commonly used as a stimulation test to assess the function of the hypothalamic-pituitary-gonadal axis in men. These tests involve a single dose 100mcg triptorelin injection, which results in quick stimulation of testosterone production.

In addition, the peptide has been employed in a similar fashion by researchers as a method to reinstate normal LH and testosterone production in subjects with a history of anabolic-androgenic steroid use [7].

Sample Triptorelin Dosage Protocol

Many researchers are interested in using triptorelin to help reinitiate baseline sex hormone levels in subjects who have been exposed to exogenous testosterone or anabolic-androgenic steroids [7].

For reference purposes, here is a sample triptorelin protocol to that end:

  • Dosage: 100mcg of triptorelin acetate.
  • Administration: Administer subcutaneously in a single dose following termination of exposure to testosterone or anabolic-androgenic steroids.
  • Research Notes: It is advised to not exceed a one-time dose of 100mcg, as a higher amount can cause side effects. Researchers may consider repeating the dose after 30 days if serum testosterone does not return to reference range during this period. Continuous (daily) use or higher doses may result in suppression of LH and testosterone synthesis.

Bacteriostatic Water and Triptorelin

Researchers need to adhere to proper methods for safe and effective experiments. When studying Triptorelin and other peptides, the mixing and storage procedures call for certain supplies.

These include bacteriostatic water and sterile vials, among other essentials. The idea of acquiring all these items may seem daunting, putting a damper on research. However, it is required.

Triptorelin Dosing | Verdict

Triptorelin is a decapeptide analog of gonadotropin-releasing hormone with a wide variety of applications.

Among its many uses, triptorelin is employed to preserve ovarian health, restore fertility, temporarily elevate testosterone production, and even restart sex hormone production after an extended period of suppression.

The peptide has a favorable safety record when properly administered and laboratory animal studies have shown that the hormone-suppressing effects of triptorelin are reversible.

Researchers looking to restart sex hormone production in suppressed subjects typically administer a triptorelin dosage of 100mcg.
