What is Sermorelin?

Sermorelin is an analogue of human growth hormone-releasing hormone (GHRH). It’s classified as a “peptide” because it’s made up of a chain of amino acids that’s not quite as long as a protein. Sermorelin contains the first 29 amino acids of GHRH, which makes it the shortest synthetic peptide to offer the full biological activity of GHRH [1].

Sermorelin is sold under the brand names “Geref” and “Gerelcan,” and can be administered either subcutaneously or intravenously. It mimics the body’s own GHRH and has the same effect, which is to increase the amount of growth hormone (GH; also known as human growth hormone or HGH) in the body by stimulating its secretion in the anterior pituitary [2].

How it works is a little more complicated. HGH is regulated in humans in a complex interaction between the nervous system and the endocrine system. The pituitary gland is especially important since this is where your body manufactures growth hormone and where it’s released [2].

Sermorelin’s chemical structure is similar to GHRH itself, so it’s able to bind to the same receptors as does GHRH. When it binds to those receptors, it tells the pituitary gland to release more HGH. Thus, the ultimate effect of sermorelin is higher concentrations of HGH in the body.

So why does this all matter? Well, HGH itself enables the body to create and repair tissue, like muscle, skin, tendons, and bones [2].

Does Sermorelin Actually Work?

It does.

Research indicates that sermorelin peptide is an effective growth hormone secretagogue that significantly increases levels of HGH in the body in both children and adults [1, 4].

Sermorelin was initially approved by the U.S. Food and Drug Administration (FDA) in 1997 and sold under the trade name “Geref” [5]. Its approval was subsequently discontinued in 2008 for reasons unrelated to its safety or efficacy [6]. It had been indicated for the treatment of growth hormone deficiency (GHD) in both children and adults and was also used as a diagnostic agent for assessing whether a given patient’s pituitary gland was functioning properly and releasing adequate amounts of HGH [3].

Is Sermorelin Legal?

Sermorelin’s legal status varies by country and jurisdiction. In the United States at least, sermorelin is not a controlled substance but it is not, however, available for recreational use. Qualified researchers may buy sermorelin online as a reference material for research purposes.

Sermorelin Before and After

So what effects might sermorelin cause in research subjects?

Since the primarily-sought effect of sermorelin is to increase the production of endogenous HGH in the body, let’s take a look at what biosynthetic human growth hormone therapy, which also increases HGH levels (albeit artificially) has been documented to achieve.

One year-long, double-blind, placebo-controlled study examined the effects of HGH supplementation in a group of men over the age of 60. Men in the group who received HGH increased their lean body mass 8.8 percent and reduced their fat mass by 14.4 percent [7].

A separate study funded by the World Anti-Doping Agency (WADA) found that GH supplementation by recreational athletes had a “significant effect” on body composition and sprint capacity. The randomized, placebo-controlled, blinded study found that participants who received GH (either alone or in conjunction with testosterone) had increased sprint capacity after eight weeks of treatment [8].

Again, it’s important to note that these two studies were not conducted with sermorelin itself, but with biosynthetic HGH. However, because the primary goal (and effect) of both therapies is to raise GH serum levels in the body, studies conducted with biosynthetic HGH should very much be on the radar of any sermorelin researcher.

Sermorelin Benefits and Uses

Our team conducted a literature review using PubMed, Google Scholar, and EMBASE to identify all relevant benefits and uses of sermorelin. Below we’ll outline what the latest research says about sermorelin benefits.

Diagnosis and treatment of growth hormone deficiency (GHD)

Sermorelin’s primary clinical use was to diagnose and treat growth hormone deficiency (GHD) in children. When administered subcutaneously, it produces a rapid hormone response [1]. Sermorelin has also shown potential as a treatment for adult-onset growth hormone insufficiency [9].

Increases in lean muscle mass

Sermorelin rapidly increases the secretion of HGH and raises GH serum levels in the body [1]. In studies conducted with biosynthetic HGH, higher GH serum levels have been linked to increased lean muscle mass. For example, elderly individuals who received biosynthetic HGH supplementation for over 12 months experienced, on average, an 8.8% increase in lean muscle mass without any changes to their diet or exercise regime [7].

Fat loss

While there are no studies directly linking sermorelin to fat loss, the peptide has been shown to increase GH serum levels. The above-cited study involving biosynthetic HGH found that elderly participants experienced a 14% reduction of adipose tissue (body fat) compared to a control group over a 12-month period [7]. As sermorelin and biosynthetic HGH have the same desired outcome (to raise GH serum levels) it is believed that sermorelin supplementation may aid fat loss.

Sermorelin Side Effects

According to a review of sermorelin’s use in the diagnosis and treatment of children with GHD, “once daily subcutaneous doses of sermorelin are well tolerated” [1]. The review noted that the most commonly reported adverse events were:

  • Transient facial flushing
  • Pain at the injection site.

According to Drugs.com, the most common sermorelin side effects are [10]:

  • Facial flushing
  • Headache
  • Nausea
  • Pain
  • Redness, or swelling at the injection site
  • Paleness
  • Strange taste in the mouth
  • Vomiting

Research participants should seek medical attention immediately if they experience any severe allergic reactions such as:

  • Rash
  • Hives
  • Itching
  • Difficulty breathing
  • Tightness in the chest
  • Swelling of the mouth, face, lips, or tongue

In terms of sermorelin’s long-term side effects, very little is known. Sermorelin is no longer approved by the FDA and there have been no long-term human clinical trials. However, a review of safety data from growth hormone manufacturers' registries spanning 1998-2016 found no evidence that GH supplementation increases the risk of the following conditions [11]:

  • Type 2 diabetes
  • Stroke
  • Leukemia
  • New malignancy

In rare cases where patients being treated with GH developed one of these conditions, pre-existing risk factors were present. Based on these findings, research subjects with risk factors for any of the above conditions should be closely monitored during follow-up.

Sermorelin Dosage Guide

Researchers interested in testing with sermorelin should know that this peptide can be dosed in a multitude of ways depending on the purpose of the trial and the age, sex, and weight of the person receiving administration.

Sermorelin acetate comes in a powder form that must first be reconstituted with bacteriostatic water by injecting the diluent into the vial of sermorelin and swirling the vial (very gently) until the contents are entirely dissolved. Vials with visible particles should be discarded and the vial should be cleansed with an antiseptic solution to prevent infection.

Once reconstituted, sermorelin can be injected subcutaneously using a sterile, disposable syringe such as an insulin syringe. It’s common to administer sermorelin at night before bed, at least 2 hours after eating, or first thing in the morning.

Unused sermorelin can be stored in a refrigerator at 2-8 degrees Celsius (36 and 46 degrees Fahrenheit) and should be kept away from light and moisture [10].

To diagnose idiopathic growth hormone deficiency in children, sermorelin is commonly administered intravenously at a dose of 1mcg per kilogram of body weight [1].

While there is strong research interest in the application of sermorelin to improve athletic performance, to date there is no published research to indicate the dosage at which this peptide may be administered to this end. This suggests that further experimentation is required in this area using either sermorelin or a sermorelin-ipamorelin blend.

Sermorelin vs. Ipamorelin

Which is better…

Sermorelin or ipamorelin?

From a research standpoint, both sermorelin and ipamorelin have significant potential for further research. Neither peptide currently has FDA approval (sermorelin’s approval was withdrawn in 2008 for reasons other than safety and efficacy [6]) and data on the clinical efficacy of either sermorelin or ipamorelin for non-medical uses such as improving body composition in healthy individuals remains lacking.

In terms of safety, ipamorelin is a selective growth hormone secretagogue that does not release ACTH or cortisol in levels significantly [12]. Researchers wary of long-term follow-up requirements for participants may view this favorably.

Both peptides are available from our preferred vendor, Peptide Sciences, at similar price points. Ipamorelin 5 mg vials ($46) are slightly more economical than sermorelin 5 mg vials ($49.50), however, in large-scale clinical trials, this price difference is negligible.

As sermorelin, a GHRH analogue, and ipamorelin, a growth hormone secretagogue receptor (GHSR) agonist, have complementary mechanisms of action, they are often blended. As we mentioned above, Peptide Sciences offers a blend of these two peptides.

Overall, both sermorelin and ipamorelin present opportunities for further research and investigation.
